I've never had that happen and ive lived in the US my whole life. Must be a regional thing. I've never gotten a check before asking for it.
Interesting, Ive had it happen in Chicago, California and Indiana. At Dennis, iHOP and some other more regional restaurants.

around 3 to 5 CHF, depending on the place, and what kind of coffee you want to drink. remember, this is Europe - the coffee is not made the same nor looks or tastes the same as you get in the US. it's mind boggling to me that people get a 16oz "cup of coffee", black.

Let me restate what the OP said. (quotes used for paraphrasing, not for quoting) "Restaurant owners set a price on their dishes, because if everyone payed at will, the owners know they would make less money. Therefore we can assume owners know that paying at-will does not optimize earnings. Therefore, it's insane that they make their staff work for pay that's at-will".

Any recommendations for tipping in India? (Amounts etc) I was warned against tipping too much by locals who worried it could distort local economies toward tourists and away from local people.
Sounds like you talked to the locals who might end up being expected to pay tips. The ones who might expect to receive tips would have a different point of view.

To be honest they are paid a full salary. Tipped wage does not preclude the minimum wage. In addition there are many states like CA that do not have a tipped wage.
