Most people are achieving weight loss by dietary changes. It takes a massive amount of exercise to significantly alter your calorie balance. Definitely good for you either way but most people who exercise also want to be toned or muscular.
Define "massive". People do successfully and reliably get in shape through exercise. Aerobic exercise that gets HR up for an hour or more will significantly alter your calorie balance. I agree that more people don't go through exercise rote though.

It does which is why medically supervised weight loss with GLP1s includes diet recommendations to mitigate this. But in my experience, decreased cravings make it easier to choose food rationally. The food noise that causes people to overeat usually doesn’t cause them to overeat healthy foods anyway.

"Food noise" is much more than just craving calories, we have cravings for most of the different nutrients we need. That is how people ate a balanced diet in history even though it was much harder to do back then, peasants craved…
I think there’s a distinct difference between what people are describing as “food noise” and a normal appetite.

the term "gut feeling" exists for a reason. the findings substantiated this colloquial wisdom and its relevance ... the article hints at it by associating mental disorders with gut microbiome. I wouldn't be surprised at all if it turns out that Ozempic et al. harm this gut/brain cooperation ... I'd bet on it.

How much and how long do you want the bet you go for? How do you even want to define "harms". Let's say there's a road from the gut to the brain. If today's ultra processed food is motorcycles ripping down the road as fast as possible, and Ozempic is the police, saying whoa there, slow down, buddy, then from the POV of the motorcyclists that's harmful, but for everyone else around them, that's good.

#507Studies show almost all subjects regained the weight and reversed gains within 2 years. This means underlying issues (e.g., food addiction) aren't being addressed. Short of changing habits, the only maintenance solution is lifelong drug use and that doesn't sit well with me.
Statins are a well understood mechanism of action for controlling cholesterol. We generally understand that people with cholesterol problems leverage statin therapy to address those, and there is a well audited corpus of placebo controlled double blind trials that demonstrate the efficacy of the solution for statin therapy. If you discontinue the statins, you lose the benefit to some extent. How is this situation fun…
I think people consciously or unconsciously consider being overweight a moral failing and so are quick to point out the flaws.

1) you didnt eat the most calorie dense foods
2) you dont need crazy calories to get fat, just above your burn rate.

I’ve been taking high doses long term for over a decade and I don’t really have any substantial issues.
The primary concern is reduced stomach acid leading to nutrient deficiencies, particularly b12. There have been some long term studies correlating to dementia as a result. And some studies that have not found this. So the jury is still out, but keeping an eye on some nutritional markers is not a bad idea.

If we can't treat the underlying condition and the patient needs life-long acid suppression therapy, we switch to H2 receptor antagonists, like cimetidine. Long term PPI use has non-trivial risks, including magnesium and other micronutrient deficiencies, and can lead to things like SIBO and other primarily gut related morbidity.
