#4I thought other reporting claimed that a ground handling truck crashed into the door, thereby detaching it?

#6Put some caution tape over the door and have the pilots fly it out empty at 6,000 feet so it doesn't have to be pressurized.

Can’t recommend the book Airframe by Michael Crichton to help contextualize the typical kneejerk reactions to these kinds of things versus the usual culprits enough.
