Big exchanges like Coinbase have centralised exchange wallets with billions of dollars, who/how many people know the private key, how do they prevent rouge employee emptying the main account?

#2No employees have direct access. The private keys should be in HSMs that have automated spending policies that require multiple approvals, etc.
