It's a long-winded argument for using checklists, later to be turned into a book and a major motion picture. (Long-winded, but worth a read, assuming you can get past the paywall, if you haven't read The Checklist Manifesto.)

Short version. Use checklists. Even intelligent and highly trained people such as pilots use them. We used them in intensive care and surgery and avoided many basic errors as a result.

I'd recommend not buying the book it's basically the above expanded to 200+ pages.
