I just tried cursor and it's not bad, but I would like to use something like this on a pay per use basis with my own claude or chatgpt api keys.

#2Maybe cline? It's a vscode extension actually, but it lets you choose the LLM.

#3I dont think it is a full equivalent, but https://github.com/continuedev/continue seems close enough. It is quite ergonomic.
