#2From my reading, the answer seems to be yes, but Apple launched mitigations already after 5 hours.

My ISP is Rostelecom and here they have the reputation of being a pretty crappy provider (probably because it's a monopoly in a lot of places), so a mistake is a possible reason, it's something Rostelecom could do.
