Packtpub Twitter Account Takeover

“The attackers had used the account hacked form/password reset form to gain control of the account through the old_twitter@packt.com email address.”

That’s the sentence that’s supposed to explain how it happened. I don’t understand this sentence grammatically. Even if I were able to decipher it, I don’t see any details in there about what happened.
