Ketones are only going to be present if you are exceptionally bad (very high carb diet) or exceptionally good (very low carb diet) at managing your blood sugar. It's not going to be terribly useful for your average diabetic who has fairly good control over their blood sugars.

This is not correct. When someone with type 1 has an illness they can develop ketones while their glucose appears stable within range. This is can lead to death. About once a year, when my son is sick, we need to monitor his ketones with finger pokes. When he has a stomach bug and can't eat, we have had to go to the hospital for a drip to allow us to give insulin and lower his ketone level. Having ketones on the CGM…

That seems like an extremely rare condition (source: I'm also a T1D and never encountered that when sick).

I am a type 1 diabetic and this is not true, and an incredible oversimplification. And categorizing "very low carb" as "good" and "very high carb" as "bad" is bizarre and untrue. You can go into DKA on a low carb diet. And low carb vs high carb for T1D is NOT a "good vs bad" equation to start with.

Im also a T1D and in my own experience high carb diets are extremely difficult to control BG with. You are chronically elevated/high or roller coastering, while with low carb diet its much easier to stabilize BG and keep sugar levels normal. With low carb diet I have achieved an a1c of 5.6 which would be impossible/dangerous with high carb diet. Dr. Bernstein's "Law of small numbers" and all. But yes, you are correct, it was a simplification.

Interesting device for those using diet to induce ketosis for diabete or mental illness care.

One great thing about the keto diet is that it is the only diet where you can be sure you follow it correctly ... by measuring your ketone levels.

And for glucose since different people will react very differently to various forms of carbohydrate (and associated foods) a CGM is always a must. (1)

Automated glucose control is already available via wearable insulin pumps combined with CGMs in a 'closed loop' system.

Minimed 780g user here, it is not perfect. For a true closed loop we need a system that can give glucagon as well. The current closed loop only gives you insulin to lower your blood glucose. When it gives too much it wakes you up and asks you to treat it. You also have to accurately count carbs of meals you eat, enter when you exercise. All closed loop systems require human action--maintenance,filling reservoirs, changing sites, dealing with the inherent variation of CGMs, estimating carbs, etc. even if glucagon delivery were on board (it's being worked on). But most are generally providing insulin delivery in real time, based on CGM data.

Also, I agree--glucagon would be a useful thing onboard for T1d, but not really for T2d, because (as you know) the essential problem there is highs, not lows.

And I definitely agree about CGM accuracy: most people don't know that there's a significant margin of error in all CGMs. It's 'close', but not precise.

A friend died of diabetic ketoacidosis. He loved technology and seeing people succeed. Thinking of him with this news. Miss you Dan. Grateful we're moving forward on so many fronts in the world.

Fasting and ketosis has nothing to do with tight blood sugar control. Your body manages blood sugar even if you're fasted or burning ketones.

Seeing just how much rice can impact your blood sugar is eye opening. Sticking to a "half the normal amount" of rice when eating Indian/Asian food can help a lot with blood sugar. And keeping blood sugar in check _seems_ to have a fair amount of researching backing up the idea that it's healthier and makes you feel better

Sure it impacts it. But what does that matter? Your body is very efficient as self regulating

It matters because high blood sugar isn't great for the body (nor is low blood sugar). A normal, non-diabetic's blood sugar might spike to 120/130 after a balanced meal. But that same person's blood sugar might spike up to 160/170 after something carb-heavy. So sure, the body corrects for it - but it would be healthier if it didn't go into that higher range at all.

There doesn't seem to be a continuous lactate sensor on the market. Lactic acid production and/or clearance are definitely useful data w.r.t. exercise (many training regimens talk about lactate thresholds) and also useful in in-patient settings. No disagreement about AGC in the hospital -- there is a lot of lab/titration work in current clinical workflows for monitoring patient glucose levels.

There doesn't seem to be a continuous lactate sensor on the market. They exist, a simple search will turn up a variety (how well they work is a different question). But see my sibling comment, they're banned from cycling competition.

To follow-up, "commercially available" products from a simple search appear to all be vaporware. I did see that cyclists have been estimating lactate thresholds through breathing rate/expiration volume measurements

Well. this seems like a big detail to leave out for something advertised as 'wearable'.

People who know about and use continuous glucose monitors already understand what "wearable" means in this context. The devices are designed to be installed by the user at home, not by a specialist. You're more likely to get a reaction to the adhesive that holds the device on your skin than the sensor filament itself.

Yeah, not being part of that market. I was viewing the headline more like an Apple Watch. That seems like the click bait of the headline, it sounds like a new feature that will be in a watch or something.
