Silly question: does it work if you refresh the page? (front-end is a one-page JS app, so the page needs to refresh)

No. When I submit, you make a call to api.rollbar.com and also in the console I get these errors: 'TypeError: resp is undefined' (line 9795) and also 'Blocked loading mixed active content "http://app-abm.marketo.com/index.php/leadCapture/save2' (line 26084). The JavaScript file is: 'https://dmmj3mmt94rvw.cloudfront.net/assets/js/app-bd80a48ab.... I hope this helps a bit more!
