I have two (both many years ago)
1. Every line of every file in the project had its line ending changed
2. Python script that called a python script that called another python script

#4A contractor placed a web service call in the onPaint() method of a .NET Windows Forms application. This blocked the paint from happening until the service call finished, causing the app to just be a blank white window.
