This is an interesting question. I don't know of any products/apps off the shelf that do this.

This google search result https://phgkb.cdc.gov/FHH/html/index.html seems interesting but the data input seems very manual and not sharable, in a digital way, across family member and doctors.

Very interesting question. I've thought for a while that a family-based CRM would be an interesting product. Maybe now need to think about adding a health module!
