Actually, they are pretty much split up. To get access to my passwords and TOTP secrets, the attacker needs one of my devices (something I have) and its password (something I know)…

Not really — I do it just for peace of mind, TBH. Although your primary password could be cracked somehow, so it doesn't hurt to have this additional layer.
