For me it will always be associated with Quake, although my first foray to world of Quake was with Quake Live. Still play it sometimes.

Cant say i have encountered this yet. But flatpak aims to solve this problem.

I think the problem is just that almost all prebuilt machines come with Windows and nobody ever gets exposed to alternatives. And most people just dont care.

Not sure what youre thinking of but older games have demos that are recorded games. Those would work well for testing nowadays too imo. Mostly performance testing but still.
